What is an ingrown toenail?

An ingrown toenail happens when the edge of the nail grows into the surrounding skin instead of growing straight outward. This commonly affects the big toe and can lead to:

  • Pain with pressure or shoes

  • Redness and swelling

  • Drainage or infection

  • Tenderness along one or both nail borders

  • Recurring flare-ups after trimming

Ingrown nails often do not improve for long without proper treatment.

Why do ingrown toenails happen?

Ingrown toenails may develop because of:

  • Improper nail trimming

  • Naturally curved or thick nails

  • Tight shoes or pressure from work boots

  • Repeated sports activity

  • Trauma to the nail

  • Swelling around the toe

  • Prior nail infections

Some patients are simply more prone to recurring ingrown nails because of nail shape.

When should you see a podiatrist for an ingrown toenail?

You should be evaluated promptly if you have:

  • Increasing redness

  • Drainage or pus

  • Severe tenderness

  • Trouble wearing shoes

  • Recurrent ingrown nails

  • Diabetes

  • Neuropathy

  • Poor circulation

  • A compromised immune system

For diabetic or vascular patients, even a “small” nail problem can become more serious if ignored.

Slant Back Procedure (Conservative Care)

For mild or early ingrown toenails, we can carefully trim and relieve the edge of the nail to reduce pressure and pain.

  • Quick, in-office relief

  • No anesthesia typically required

  • Best for early or first-time cases

  • May not prevent recurrence

Simple Toenail Avulsion

For more painful or infected ingrown nails, the problematic portion of the nail is removed.

  • Performed under local anesthesia (you won’t feel pain)

  • Immediate relief of pressure

  • Helps resolve infection and inflammation

  • Nail will regrow over time

Permanent Solution for Ingrown Toenails

P&A Procedure (Permanent Solution)

This is a quick, in-office procedure designed to provide immediate relief.

For recurring or chronic ingrown toenails, a partial nail avulsion with permanent correction (P&A) may be recommended.

During your visit:

  • The toe is fully numbed with a local anesthetic (you will not feel pain during the procedure)

  • The ingrown portion of the nail is carefully removed

  • The root of that nail edge is treated to prevent it from growing back

  • The entire procedure typically takes 15–20 minutes

After the procedure:

  • You leave the office the same day with a simple bandage

  • You can wear a loose-fitting shoe (athletic shoe or sandal)

  • You are able to drive yourself home

  • Most patients return to work the same day and feel significant relief almost immediately.

Not sure which treatment you need?

Dr. Johnston will evaluate your toenail and recommend the most appropriate option based on:

  • Severity

  • Infection

  • Recurrence history

  • Overall health (including diabetes or circulation concerns)

Tired of recurring ingrown toenails?

If the same nail keeps becoming ingrown, a permanent nail border procedure may be the best option.

This treatment is designed to:

  • Stop repeat ingrown nail episodes

  • Reduce future pain and infection

  • Narrow the nail slightly

  • Improve long-term comfort

For many patients, this is the most effective long-term solution.

What to expect after treatment

After treatment, most patients can return home right away.

Depending on the procedure performed, recovery may involve:

  • A bandage

  • Simple soaking or wound care instructions

  • Temporary tenderness

  • Open-toe or roomy shoes for comfort

  • Follow-up if needed

We will give you clear aftercare instructions to help healing go smoothly.
